  • Mike Potter, who did Hedwig's hair and makeup for both the stage production and the movie, can be seen in drag in the scene where Tommy picks Hedwig up off the street, as the second call girl leaning against the wall. He was given the drag name "Renatta Options Options" by John Cameron Mitchell.
  • During a montage of Hedwig giving Tommy "rock" lessons he is seen wearing a "Hotel Riverview" t-shirt. The Hotel Riverview, NY, was the original location of the stage show for many years.
  • The scene in which Hedwig walks in on Yitzhak wearing the wig, the song "Random Number Generation" is playing in the background. This song is from the original stage show
  • Stephen Trask, who provided the singing voice for Tommy, was the one who wrote the songs and score for the film.
  • The cape that Hedwig wears mimics the Berlin Wall. It is complete with "Yankees go home" on the right side and "With me" on the left.
  • The Bilgewaters chain of restaurants are decorated with graphics of the Titanic sinking. Hedwig and the Angry Inch perform here on several occasions, and the original stage production of the show took place in a ballroom of a hotel that was used as a shelter for the surviving crew of the Titanic in 1912.
  • James Cameron Mitchell makes a cameo appearance as the reporter who shouts out to Tommy saying "Tommy, what's your relationship to Hedwig Robinson."
  • The aqua and peach track jacket worn by Hedwig's make-up man in the call girl in that alley scene shows up several times throughout the film as an inside joke.
  • Because James Cameron Mitchell was required to wear so much make-up for long periods of time, he would go to his trailer during lunch breaks to shave off his five o'clock shadow and have his make-up touched up again.
  • Unlike most musical films the films musical segments are only partly filmed on playback. John Cameron Mitchell didn't want to have to lip-sync the lines in the music so the band playing was recorded in studio, played back on set and sang live. His vocals were recorded and then mixed later on with the recording of the band, making it sound like they too were performing live. However, on the soundtrack, the vocals were mixed to sound as if they were recorded in a studio as opposed to on set.
  • Yitzhak is seen wearing a "Rent" sweatshirt and decides to audition for the role of Angel in Rent. James Cameron Mitchell was originally offered the role of Angel, but had to turn it down because he was performing as Hedwig at the same time.
  • Maggie Moore who played the woman singing "Dolly Parton's "I Will Always Love You," played the role of Yitzah in the stage production once Miriam Shor left the role.
  • Hedwig picks up a photo of herself as a young boy in the "Wig in the Box" song, and the title can be seen as "Edge of Awareness."
  • The photo that is taken right after Hedwig and Tommy are arrested show as date of 8/13/01, 40 years exactly after the Berlin wall was erected.
  • The footage from "The Rosie O'Donnell Show" (1996) was actually filmed when she is promoting the Off-Broadway musical. The Hedwig CD that is held up is actually the original Broadway cast recording.
  • The stage version played at the Jane Street Theatre Off-Broadway for over two years.
