The film opens with a flashback to Captain Elliot Spencer fiddling with the Lament Configuration puzzle box, solving the riddle of its ornate faces and opening an interdimensional portal to a hellish plane of existence. Pulled through this tear in the fabric of space-time he is transformed into the Cenobite called "Pinhead". Back in present times, Kirsten Cotton is institutionalized at the Channard Institute after her previous run in with the box that killed her entire family. She meets another patient, Tiffany, who has a gift for puzzles, but soon after their first meeting Kirsty has trouble sleeping as strange visions of her father calling for help from the pain dimension dominate her dreams.
As it turns out, the founder of the mental hospital Kirsty is staying at, Dr. Phillip Channard, holds an unhealthy obsession with the Lament Configuration box and he's become intent on resurrecting Julia in this dimension. To that end he begins bringing patients from the hospital for her to feed on, but when Channard's assistant, Kyle, discovers that Kirsty has been telling the truth about the box's evil the two go to confront the mad doctor. By the time they arrive, though, Julia is almost fully regenerated and she kills Kyle and incapacitates Kirsty while Channard has kidnapped Tiffany so she can unlock the box. Kirsty wakes up and crosses dimensions in hot pursuit.
In the other dimension Julia betrays Channard to the Cenobites and he is turned into one himself. Kirsty tries to escape with Tiffany and runs to a house she thinks is her father's, only to discover Frank there. When Julia arrives she rips Frank's heart out allowing Kirsty and Tiffany a narrow escape back to the hospital. Channard, now a Cenobite, is right on their heels and the pair run into not only him, but the entire group of original Cenobites led by Pinhead. Kirsty knows that they used to be human, though, and she appeals to their former humanity to convince them to fight against Channard, but the former doctor is too powerful and he dispatches the four in moments. With Channard about to also kill Tiffany, Julia (actually Kirsty disguised) arrives to distract him just long enough for Tiffany to solve the puzzle and kill the Cenobite, saving their lives.
Months later movers arrive at Channard's house to pack up his things, not noticing the bloodied furniture. Suddenly chains fly out to kill the two men and a pillar rises in the middle of the room showing the faces of Pinhead, Julia and the Moroccan vendor from the first movie, asking again "What's your pleasure?"
