“Vengeance, the colder it gets, the more venomous it is; Vengeance, the hotter it is the more brutal it gets”
On a snowy night, Ju-yeon Oh (San-ha Oh) is stuck in the middle of a deserted road with a flat tire. While speaking with her finance Su-hyeon Kim (Byung-hun Lee) over the phone, she is approached by Kyeong-cheol Jang (Min-sik Choi) who offers her a ride which Ju consistently refuses. Jang smashes her window and brutally beats her over the head with a hammer until she loses consciousness. Jang then drags her body into his van, drives her to his hiding place, kills her and chops up her body and scatters the pieces on the roadside.
A boy discovers one of Ju’s ears the next day which leads to the rest of her body parts. Su sees the body and blames himself for failing to protect his fiancée . His grief becomes vengeance as he is determined to track down Ju’s killer and avenge her death no matter the cost.
At her funeral, Su swears that he will return Ju’s pain to her killer 1000 times fold. Due to his job as an agent of the National Intelligence Service (NIS), Su has access to high-tech equipment, including a GPS capsule tracking device with a microphone which he ask his colleague to steal from the agency in order to exact his revenge. Using the connection with his would be father-in-law, Chief officer Oh, Su has a list of four suspects in Jun’s murder. One by one Su tracks down the suspects and brutally interrogates them. At one point, Su repeatedly hammers a suspects groin with a wrench in order to make them confess their own crimes which are not related to Ju’s murder. Finally Su receives Jang’s location by tracking down his estranged family including Jang's son whom he has abandoned and is living with his grandparents.
Upon investigating Jang ’s residence, Su finds his fiancée ring near the sewers, confirming Jang as the killer. Su then waits for Jang to return from his day job as a bus driver of an all-female high school. Su finds Jang in the middle of taunting and assaulting one of the high school girls. Given Su’s martial art’s training, he quickly subdues Jang and renders him unconscious with a plastic bag over the head. However, Su has no intention of killing him quickly Instead he savagely beats him, force the GPS capsule down his stomach, breaks his hand and leaves him on top of an open grave.
Secretly tracking Jang , Su follows him to the hospital where he gets his arm fix, and attempts to sexually assault the nurse that was helping him. Su intervenes and beat up Jang again, and promptly cut off his Achilles tendon.
After Jang regains conciseness, he takes refuge with his friend Tae-joo (Moo-sung Choi) who is into cannibalism. During dinner with Tae and his “girlfriend”, Tae suggests that whoever is hunting Jang must have ties to one of his previous victims. After Jang sexually assaults Tae’s “girlfriend”, she is dragged into the kitchen by Tae and is about to be butchered when the music player, which has been playing “Carmon” the whole time, is stopped by Su who overpowers Tae and is about to kill him, before he is interrupted by Jang . Using his acrobatic skill and training, Su is able to subdue both men.
Su and Jang are taken to a private medical facility where Jang , while unconscious, learns about the GPS capsule in his stomach. After Jang is released, he taunts Su via the capsule and tells Su that he knows who Su is and where his fiancee's family lives. To prevent Su from tracking him, Jang defecates the capsule out by ingesting large amount of laxative. Jang then proceed to Ju’s residence where he injures her father and sexually assaults and murders her sister.
Su races to Ju-yeon’s home but it’s too late, Jang tells Su that he is going to give himself up to the police, thus eliminating any chance for Su's revenge. However,
before Jang can give himself up, Su rushes in with a car and captures Jang before the police arrive. Su drags Jang to his parents house where he has set up a Rube Goldberg decapitate machine with the guillotine switch tied to the door. Jang ’s son, unaware of the trap opens the door and kills Jang. Su, listening to Jang's final agony, finally gets his revenge, and falls onto the sidewalk crying.
