I was wondering this the other day when I was watching Queer as Folk, because there are scenes when they explicitly show Brian Kinney fucking Justin Taylor, and I just thought it would be quite awkward if they were actually doing it, especially considering that the actor who plays Brian (Gale Harold) is actually straight.
You know, it's funny - I just watched Chloe Sevigny's fellatio scene in The Brown Bunny (never heard of it before reading this thread) and yeah it was a...blowjob. Anyways, I remember her saying that she had spoken with a friend of hers whom she had always been sexually attracted to, asked the guy's wife if she was okay with the dood and Angelina boning, and the wife had said yes. Essentially, it was all a way to lead up into this whole rumor mill that her and Antonio actually boned in the movie, trying to bring up ticket sales, yadda yadda.
Then they caved and said "nah, it was fake".

In a 70's classic action movie (can't remember the name), the actors have 2 sex scenes (at least, I think), in the 1st scene, they are at a normal position, then the actor moves the legs of the actress towards/against she (you now, "chicken position") and "get in", either by accident or by his own, then we see that the actress get surprised and try to stop the actor, but he "accelerates", then she relax and she get an orgasm (maybe because she is not waiting, trying to get him "out",...), really, it's not a simulated scene, because the behaviour of them in the movie changes after this scene (when they are filming together, you note that they are "different").
Also, in the majority of Brazilian Films, I think that they really "do the thing", or they are the best ever actors, because the scenes appears to be "trully" real.
