Here is the ass-to-mouth horror comedy you never asked for.

Set in rural Germany, a psychotic surgeon drugs and kidnaps two Americans and a Japanese tourist, then sews their mouths to the other's anus, creating a human centipede. Ludicrously gruesome horror.

Plot

During a stopover in Germany in the middle of a road trip through Europe, two American girls, Lindsay and Jenny, find themselves alone at night when their car breaks down in the woods. The girls wander through the forest and find a villa, where they are welcomed in by Dr. Heiter. After faking a phone call to road side assistance, Heiter drugs the girls and brings them into his basement laboratory. Heiter has also kidnapped a trucker, but he is not the right "match" for Lindsay and Jenny, so he kills him. Lindsay unties her bindings and almost manages to escape, but Heiter shoots her with a tranquilizer dart.

When the girls wake up there is a new captive with them, Katsuro a Japanese man. The doctor proceeds to inform the captives of his plans. That his expertise is in detaching conjoined twins, but his dream is doing the opposite. By severing the tendons in their knees, and attaching them to each other through the gastric system, he will create a Human Centipede.

Heiter completes the operation, and treats the centipede like a pet. He tries to get it to perform simple tasks like fetch, but Katsuro, who is the head of the centipede, refuses and is beaten. Before long Katsuro has to defecate, and Lindsay who is the middle piece must swallow his excrement. While Heiter is examining the centipede, he finds that Jenny is dying of blood poisoning, due to excrement getting into her facial wounds. Two detectives, Kranz and Voller, come to the villa to question Heiter on the disappearances of some tourists. After Heiter loses his temper with them, they leave to get a search warrant for his home. .

Heiter runs down to the basement to inform the centipede that he will replace Jenny with two new additions, but Katsuro stabs him in the leg with a scalpel and tries to escape. As they are trying to break a window, Heiter crawls up behind them. Katsuro approaches him with a shard of glass, however he starts saying that he deserved to become an insect because he left his family, then plunges the glass into his own neck and dies. Meanwhile the detectives have returned, and break into the house while Heiter crawls into the room with a swimming pool. Kranz is shocked when he discovers the centipede, but before he can help a gunshot is heard. Kranz runs towards the sound and finds Voller face down in the pool, and does not notice Heiter until he is shot twice in the chest by him. Before dying Kranz manages to shoot Heiter between the eyes. Jenny dies of blood poisoning, leaving Lindsay attached to two corpses.

Development

The idea for the film was originally conceived by Tom Six as a joke. When Tom would see a pedophile in movies or on the news, he would always say to his friends that "They should stitch his mouth to the ass of a very, very, fat truck driver as punishment." His friends all thought it was disgusting, so naturally Tom figured it would be a great concept for a horror film.

The tagline for the film, "100% Medically Accurate", is not meant as a joke. Tom truly wanted the operation in his film to be authentic. He contacted a Dutch surgeon and approached him with the idea of creating a real human centipede procedure. The surgeon at first thought Tom was crazy, saying it would violate this professional oath, but after reading the script and being a movie lover himself the doctor agreed. He drew up medical diagrams for the procedure which the special effects team followed in their creating of the centipede. The drawings you see in the scene where Heiter explains the procedure, are the actual drawings that the Dutch surgeon came up with.

Tom said the choice for Dieter Laser (best name ever) as Dr. Heiter was obvious. He had seen Laser in a number of films and loved him for his "beautiful face and beautiful voice." Laser loved the idea of playing the doctor and immediately agreed to star in the film. The casting of the centipede parts was a little more difficult. They had the casting call in New York City, and a lot of the girls who showed up for the role not knowing about the content of the movie immediately left in disgust. Ashley C. Williams and Ashlynn Yennie were two of the actresses brave enough to get on their knees and place their faces near each others asses, for the casting positioning. Akihiro Kitamura was a little easier to cast, as he immediately loved the idea of having a woman attached to his ass.

Sequel(s)

Tom Six announced a second film, The Human Centipede II (Full Sequence). He said that The Human Centipede II would be "unwatchable" and would make the first one "My Little Pony" compared to the first. It was released in 2011.

Tom Six also announced a third film, subtitled "Final Sequence", saying that it would make the already incredibly graphic Full Sequence look like "a Disney film" and that it would "make a lot of people upset".
