After being horribly disfigured,(having acid thrown on his face is the most common happening), District Attorney Harvey Dent took on the name of "Two-Face" and set out to punish those responsible. He is known to carry a double-headed coin, with one side scratched up.

Harvey Dent was one of Gotham City's District Attorneys and leading the fight against crime in Gotham City. He used is partnership with Batman and Commissioner Gordon to lock up or foil the plans of some of Gotham's biggest criminals. Until, he pissed off the wrong guy.
 
 
 
 
 
 

The Disfigurement


Harvey was given his disfigurement by the hands of Gotham's most notorious criminals in one way or another. 
 
In 1995's Batman Forever, Harvey has acid thrown on his face by mob boss Sal Maroni (this is origin story of Two-Face in the comics). He blamed Batman for not rescuing him in time. 
 
In 2008's The Dark Knight, Harvey is kidnapped and tied up in a warehouse full of explosive barrels. His girlfriend is in the same situation in a different location. As Harvey attempts to escape the ropes tied around him, he falls over and half of his face falls into a puddle of gasoline. Batman accidentally comes to save Dent (after being told the wrong location by The Joker). As Batman and Dent are escaping, Harvey pleads with Batman to leave him and go save Rachel. As Batman tries to carry a struggling Dent outside, the warehouse explodes and a flame catches the side of Harvey's face that fell into the gasoline.

Batman: The Brave and The Bold


In one of Batman's hallucinations, Two-Face and his gang capture Batman, but Batman pints out he can't kill him unless he flips his coin. He flips it and the coin lands good and before his goons can kill him, Batman takes them down, then Two-Face explains that the first toss was to determine if his henchmen would kill Batman and the second was to decide if he did it himself. When Two-Face flips his coin a second time, Batman knocks him out before it falls down to his hand.
